We measure the success of our own work, our partnerships and our social investments by how they make progress towards our bridge building outcomes and how they achieve our aims.

Successful applicants are required to complete a Monitoring Form to report on the social return on our investment. Current grant-holders can download the monitoring form here. (Please note you will need your application reference number).

• Download the Monitoring Form for grants of £5,000 or less and for any grant made before September 2008.

• Download the Monitoring Form for grants above £5,000 (in total) made from September 2008 onwards.
